Official Membership, Waiver, and Release Form
In order to become a member of the JSVBA, everyone must fill out the Membership, Waiver, and Release Form.
This form can be completed on-line, mailed in, or you may bring the completed
Off-Line form with you to your first event. We will also have the
form available for completion at the event during registration.
 On-Line Membership Form
[Click Here]- Interested in joining the JSVBA? Well, it's easy. Just fill out
the membership form and click the Submit Button. We'll get you registered and ready to play.
You must be 18 or older to apply on-line.
Off-Line Membership Form [Click Here]- Under 18? Then we need your parent/guardian's signature.
That's easy too. Just fill out the Off-Line membership form,
get it signed and bring it with you to your first event.
