[Click Here]- In order to become a member of the JSVBA, everyone must either
apply online or print the following Off-Line form and bring it to an event.
You must be 18 or older to apply On-Line.

- In order to become a member of the JSVBA, everyone must either
apply online (above) or print and fill out this Membership, Waiver, and
Release Form. If you are under 18 your Parent/Guardian needs to sign
this form and then you can bring it with you to your first event.
